مقایسه اثربخشی درمان فراتشخیصی یکپارچه و واقعیت درمانی بر تحریک‌پذیری و خودکارآمدی در روابط اجتماعی با همسالان در کودکان دارای اختلال بی‌نظمی خلق اخلالگر

چکیده

مقدمه: پژوهش حاضر با هدف مقایسه اثربخشی درمان فراتشخیصی یکپارچه و واقعیت درمانی بر تحریک‌پذیری و خودکارآمدی در روابط اجتماعی با همسالان در کودکان دارای بی‌نظمی خلق اخلالگر انجام شد.
روش: روش پژوهش نیمه آزمایشی با طرح پیش‌آزمون، پس‌آزمون و پیگیری با گروه کنترل بود. جامعه آماری پسران ۸ تا 11 سال دارای اختلال بی‌نظمی خلق اخلالگر شهر اصفهان به همراه مادرانشان بودند که در سال 1401 به مراکز تخصصی مشاوره کودک شهر اصفهان مراجعه کردند. از میان آن‌ها 45 کودک به همراه مادرانشان به شیوه هدفمند انتخاب شده و در سه گروه ۱۵ نفره به صورت تصادفی قرار گرفتند. ابزارهای پژوهش شامل مقیاس اختلال کژتنظیمی خلقی ایذایی شاه محمدی و همکاران، شاخص واکنش‌پذیری عاطفی استرینگرز و همکاران و مقیاس خودکارآمدی در تعامل با همسالان ویلر و لد بود. بر روی گروه‌های آزمایش مداخلات (درمان فراتشخیصی و واقعیت درمانی) طی 10 جلسه 60 دقیقه‌ای برای کودکان و 30 دقیقه‌ای برای مادران اجرا شد. داده‌های پژوهش با تحلیل واریانس با اندازه‌‌گیری مکرر مورد تجزیه و تحلیل قرار گرفت.
یافته‌ها: نتایج نشان داد درمان فراتشخیصی یکپارچه و واقعیت درمانی منجر به کاهش تحریک‌پذیری و افزایش خودکارآمدی در روابط با همسالان شده و این اثربخشی معنادار و با ثبات بوده است (0/001>p). بین دو روش درمانی تفاوت معنی‌داری وجود نداشت.
نتیجه‌گیری: بنابرنتایج از هر دو درمان مذکور می‌توان به منظور بهبود تحریک‌پذیری و خودکارآمدی در روابط اجتماعی با همسالان کودکان دارای اختلال بی نظمی اخلالگر در مراکز تخصصی مشاوره کودک استفاده کرد.

This study aimed to compare the effectiveness of unified transdiagnostic treatment and reality therapy on irritability and children’s self-efficacy for peer Interaction in children with disruptive mood dysregulation disorder.
Methods: The study was quasi-experimental, utilizing a pre-test, post-test, follow-up design, and included a control group. The statistical population consisted of 8-11 year old boys with disruptive mood disorder in Isfahan city along with their mothers who referred to child counseling centers in Isfahan city in 2022. Among them, 45 children and their mothers purposively selected and randomly assigned to three groups of 15 (two experimental groups and one control group).The research tools included Shah Mohammadi et al.'s Disruptive Mood Dysregulation Disorder Scale, Stringer et al.'s Affective Reactivity Index, and Wheeler and Ladd's peer self-efficacy scale. Interventions (transdiagnostic treatment and reality therapy) were implemented for the experimental groups during 10 sessions, with 60 minutes for children and 30 minutes for mothers. Research data were analyzed using repeated measures analysis of variance.
Results: The results showed that unified transdiagnostic treatment and reality therapy led to a decrease in irritability and an increase in Children’s Self-efficacy for Peer Interaction, and this effectiveness was significant and stable (p<0.001). There was no significant difference between the two treatment methods.
Conclusion: As a result, both mentioned treatments can be used to improve irritability and self-efficacy in relationships with peers of children with disruptive disorder in specialized child counseling centers.
