اثربخشی روان درمانی انتقال محور بر کنش تأملی و کاهش نشانه شناختی بیماران مبتلا به اختلال شخصیت مرزی

مقدمه: پژوهش حاضر با هدف تعیین اثربخشی روان­درمانی انتقال­محور بر کنش تأملی و کاهش نشانه­ شناختی در مراجعانِ مبتلا به اختلال شخصیت مرزی صورت گرفت.
روش: هدف پژوهش، کاربردی و طرح آن نیمه­آزمایشی، از نوع A-B-A بود. ازمیان مراجعانِ دارای اختلال شخصیت مرزی که به بیمارستان ابن­سینا­ شهر مشهد رجوع کردند، 3 نفر به­روش نمونه­گیری در دسترس انتخاب شدند و به مدت یک­سال تحت روان­درمانی انتقال­محور قرار گرفتند. داده­ها در سه مرحله­ی خط­پایه، درمان و پیگیری طی 15 ماه، بااستفاده­از پرسشنامه­های کنش تأملی و پرسشنامه صفت اسکیزوئیدی (نسخه­ی B) جمع­آوری شد. برای تحلیل نتایج از شاخص­های تحلیل دیداری، درصد بهبودی، اندازه اثر و تغییر پایا استفاده شد.
یافته‌ها: نتایج نشان داد که در مراحل درمان و پیگیری، نمره­ی کلی کنش تأملی، افزایش و نشانه­های مرزی، کاهش معنادار داشت. شاخصِ اطمینانِ کنش تأملی، افزایشِ معنادار نشان داد، حال­آنکه شاخصِ عدم­اطمینان، تغییر معناداری نداشت. بعلاوه، شاخص­های تحلیل بیانگرِ کاهش معنادارِ ناامیدی و تکانش­گری بودند، منتها این تغییر برای نشانه­ی تجزیه ­و­ پارانویا مشاهده نشد.
نتیجه‌گیری: روان­درمانی انتقال­محور می تواند در مدت زمان یک­سال موجب افزایش کنش تأملی و تغییرات ساختاری و نشانه­ شناختی مهم در بیماران مرزی شود. پژوهش­های بیشتر درخصوصِ ابعاد ژرف­تر درمان نیاز است.

Introduction: The present study aims to determine the effectiveness of a transference-focused psychotherapy (TFP) on reflective functioning (RF) and symptomatic reduction in clients with Borderline Personality Disorder(BPD).
Method: This applied research was a semi-experimental, single-subject A-B-A design. Among the clients with BPD who referred to Ibn-e-Sina Hospital in Mashhad, 3 persons, were selected by purposeful sampling, and were subjected to TFP for one year. Data were collected using (RFQ) and Borderline Personality Questionnaire (STB) in three stages of baseline, treatment and follow-up in 15 months.
Results: The findings revealed significantly increased  RF total scores and decreased BPD symptoms in the treatment and follow-up stages. Reflective function of certainty has been significantly increased whereas no change detected for uncertainty. Further, a significant reduction in hopelessness and impulsivity observed, but this changes was not evident in paranoia and dissociation symptoms.
Conclusion: The one year TFP treatment could effectively increase the reflective functioning and create important structural and symptomatic changes in the BPD patients. Future research is required for deeper understanding of the treatment dimensions.
