مقایسه اثربخشی درمان شناختی-رفتاری و آموزش تنظیم هیجان بر تحریک پذیری، افسردگی، اضطراب، استرس و تنظیم هیجانی نوجوانان دارای اختلال بدتنظیمی خلق مخرب

چکیده
مقدمه: اختلال بدتنظیمی خلق مخرب با تحریک‌پذیری و حملات خشم مکرر مشخص می‌شود. هدف پژوهش حاضر مقایسه اثربخشی درمان شناختی‌رفتاری و آموزش تنظیم هیجان بر تحریک پذیری، افسردگی، اضطراب، استرس و تنظیم هیجان در نوجوانان دارای اختلال بدتنظیمی خلق مخرب بود.
روش: این پژوهش به‌صورت یک مطالعه نیمه‌آزمایشی (پیش‌آزمون و پس‌آزمون همراه با گروه کنترل) طراحی و اجرا شد. جامعه آماری آن را کلیه دانش‌آموزان دارای نشانه‌های اختلال بدتنظیمی خلق مخرب پسر شهر شاهرود در سال تحصیلی 97-96 تشکیل دادند. نمونه این پژوهش شامل 45 دانش‌آموز پسر دارای نشانه‌های اختلال بدتنظیمی خلق مخرب بود که به شیوه هدفمند انتخاب شدند. برای جمع‌آوری داده-ها از مقیاس شاخص تحریک‌پذیری عاطفی، تنظیم هیجانی و مقیاس اضطراب، افسردگی و استرس استفاده شد.
یافته‌ها: نتایج نشان داد که درمان شناختی‌رفتاری و تنظیم هیجان به طور معنی‌داری کاهش افسردگی، اضطراب و تحریک‌‌پذیری اثربخش بوده‌اند. نتایج آزمون تعقیبی نشان داد میانگین نمرات گروه‌های آزمایشی در متغیرهای مورد مطالعه به طور معناداری کمتر از گروه کنترل بود. آموزش تنظیم هیجانی نسبت به شناختی‌رفتاری بطور معنی داری در کاهش علایم افسردگی موثرتر بود.
نتیجه گیری: نتایج حاکی است که درمان شناختی‌رفتاری و آموزش تنظیم هیجانی می‌تواند افسردگی، اضطراب و تحریک‌پذیری نوجوانان را تعدیل نماید.

Abstract
Introduction: Disruptive mood dysregulation disorder (DMDD) is characterized by irritability and frequent anger attacks. The purpose of this study was to compare the effectiveness of cognitive-behavioral therapy and emotion regulation training on irritability, depression, anxiety, stress and emotion regulation in adolescents with DMDD.
Method: This study was designed and conducted as a semi-experimental study (pre-test and post-test with control group). the statistical population was all students with symptoms of DMDD in Shahroud in the year 96-97. The sample of this study consisted of 45 male students with symptoms of DMDD who were selected in a purposeful manner. Emotional Excitement Index, Emotional Adjustment Scale, and Depression Anxiety Scale were used for data collection.
Results: The results showed that cognitive behavioral therapy and emotion regulation training were effective in reducing depression, anxiety and irritability. The results of the post hoc test showed that the mean scores of the experimental groups in the studied variables were significantly lower than the control group. Emotion regulation training significantly was more effective in reducing depressive symptoms than cognitive-behavioral therapy.
Conclusion: The results suggest that cognitive behavioral therapy and emotional regulation training can modulate depression, anxiety and irritability in adolescents.
